Etanercept

Etanercept is a biologic drug that blocks tumor necrosis factor (TNF), a key protein driving inflammation in several autoimmune diseases. Structurally, it is a fusion protein consisting of the p75 TNF receptor linked to the Fc portion of human IgG1, which allows it to bind and neutralize TNF-alpha both in the bloodstream and at sites of inflammation. By acting as a "soluble trap" for TNF, etanercept prevents this inflammatory cytokine from binding to its receptors on cell surfaces, thereby dampening downstream inflammatory cascades that cause joint swelling, tissue destruction, and skin lesions.

Approved by the FDA in 1998, etanercept was the first TNF inhibitor to reach the market, marking the beginning of the biologic era in rheumatology. It is indicated for moderate-to-severe rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, juvenile idiopathic arthritis, and plaque psoriasis. The drug revolutionized the management of autoimmune disorders by offering targeted disease-modifying therapy; patients treated with etanercept experience rapid symptom improvement and reduced long-term joint damage—outcomes that were unimaginable in the era of gold salts and methotrexate alone.
